The San Francisco Public Utilities Commission has created a Home Water Conservation Checklist to work through some of the opportunities and highlight good solutions. The first item on the list is the toilet, because while not a sexy thing to upgrade, making old toilets more water-efficient is a quick way to reduce overall water usage.
